El cuarto Doctor es, para muchos whovians, el mejor Doctor de la historia, y no es para menos... Estuvo 7 años interpretando al Señor del Tiempo (1974-1981) y es, por ahora, el Doctor más longevo de la serie. Su célebre, colorida y kilométrica bufanda es uno de los iconos mas conocidos de Doctor Who, al igual que las gominolas o K-9, al que conocemos en su época como Doctor.

El cuarto Doctor es sin ninguna duda el Doctor mas reconocido en todo el mundo. La época dorada de Doctor Who empieza con él. Seriales tan memorables como Genesis of the Dales, City of Death, Pyramids of Mars o Logopolis pertenecen a sus años como el Doctor.

Se puede notar un notable cambio cuando comienza su última etapa como Doctor, en los 80. Cambia el vestuario, una bufanda aun más larga y en tonos rojos y granates, al igual que su abrigo, se añaden los interrogantes en el cuello de la camisa que le acompañaran hasta su sexta encarnación y las historias pasan a ser más de ciencia ficción que antes.

También en la música se aprecia un cambio muy importante, empezando por el intro. Por primera vez se cambia el tema realizado por Delia Debyrshire, que llevaba escuchándose desde 1963, por una nueva versión interpretada por Peter Howell. También se crea música mucho más compleja gracias a la modernización de la BBC Radiophonic Workshop.
